Job Role: Supported Housing Worker
Salary: £24,500 per year
Hours: 37.5 hours per week
Contract type: Permanent
Location: Somerset East & Somerset (mainly Frome & Shepton Mallet)
Additional information:
- Valid driver's license and access to a car business purposes
- Participation in an on-call, out of hours rota (£100 per week when on-call)
- Occasional weekend work may be required
The Role:
As a Support Worker, you'll provide personalised, high-quality support to individuals who have experienced rough sleeping and have high support needs. You'll work across four small housing units, building strong relationships with residents to help them gain stability, overcome challenges, and move towards greater independence. With an average caseload of just four clients, you'll have the time to provide meaningful, one-to-one support.

Responsibilities include:
- Progress and track referrals into the service and outcomes for clients moving on from the service
- Complete holistic assessments for clients, leading to the development of support plans which meet individual client goals
- Give clients advice, and information and provide an enabling service so that they can be signposted to and access appropriate services to develop independence
- Embed client involvement in all aspects of the support we provide and create a culture within service delivery which ensures the clients voice and feedback is encouraged, and heard
- Maintain client records that they are accurate and fully updated using our client record system
- Since job descriptions cannot be exhaustive; the post holder may be required to undertake other duties, which are broadly in line with the above key responsibilities
Qualifications / Requirements:
- A valid UK driving licence and access to own vehicle – business insurance will be required for roles involving travel for work
- Experience working with vulnerable adults and/or people with complex needs and challenging behaviours
